The genetic basis of the Healthy Blood Donor Effect (HDE)

New research from the European Journal of Human Genetics sheds light on the “Healthy Donor Effect” — and the findings go far beyond eligibility criteria. A large study of more than 53,000 active blood donors and 228,000 controls reveals that donors carry genetic variants that protect against a wide range of diseases, including anemia, autoimmune disorders, mental health conditions, infections, and even Alzheimer’s disease.

The authors note: “Blood donors are genetically protected against several diseases beyond those affecting donation eligibility.” They also identify 46 significant loci, with 25 showing direct genetic effects on long-term donorship — reinforcing that sustained donor health is influenced by both genetics and behavior. For diagnostics companies, biobanks, and health system leaders, this study highlights a broader truth: genetic architecture, population selection, and phenotype enrichment all shape real-world clinical data — and market strategy must account for it.
